The Four Things That Push a Door Off the Rails
A vehicle nudging the door is the obvious one. The others are quieter. A snapped cable drops one corner and the rollers on that side come out. Worn rollers with sloppy stems wander until one climbs the track lip. A bent horizontal rail, usually from something stored against it, gives the roller somewhere to escape.

Bind hard enough and the opener force setting overrides it, and something has to give. On a door with one roller per section per side, that is usually the rail.
Bracing, Correcting, and Proving It Runs Straight
First the sections get supported and the springs clamped so nothing can move. Rollers are worked back into the track one at a time from the top down, not forced. Bent rail is straightened only where the bend is gentle. A kinked or creased track gets replaced, because a crease will derail the door again.
Then the reason it happened gets fixed: a cable renewed, a drum reseated, worn rollers swapped, a loose jamb bracket relagged into solid framing. The door is cycled by hand, then under power with the force settings rechecked. Track spacing gets set so the rollers sit about three quarters engaged.
